## Summary

NHS Shared Business Services Limited intends to establish a Population Health Management framework for NHS and social care organisations, other approved public sector bodies and authorised private sector users. It will cover software solutions that analyse and manage health data from multiple sources to improve healthcare delivery, decision-making, prevention, efficiency and action on health inequalities and wider social determinants. The planned structure has three lots: digital population health management solutions; consultancy and support; and combined solutions providing bundled services or comprehensive workflows. Across the framework, suppliers will provide software licences or subscriptions, implementation, configuration and integration, training, onboarding, change management, maintenance, updates, technical support, service levels and performance reporting. Delivery is intended across the United Kingdom, Guernsey, the Isle of Man and Jersey.

This procurement is at the planning stage and is currently a preliminary market engagement exercise, not an invitation to tender. NHS SBS expects to use the Procurement Act 2023 and establish a framework agreement. It is seeking market views on capability, innovation, interoperability, pricing, barriers to entry, lotting and service specifications. Participation may involve a questionnaire, virtual briefing and optional one-to-one discussions; responses will inform the final specification, procurement strategy and route to market and will not form part of later evaluation. Suppliers are invited to submit the market-engagement questionnaire through SAP Ariba by 12 noon GMT on 7 October 2026. The anticipated framework period is 14 August 2028 to 13 August 2032. Its estimated value is £100 million excluding VAT, or £120 million including VAT.

The work will suit established population health technology providers, healthcare data and clinical information specialists, software developers, implementation partners, technical support providers and consultancies advising NHS, public health, social care or local government organisations. Lot 1 favours scalable platforms for whole or large populations, drawing data from multiple organisations and offering broad functionality, horizontal integration and potentially specialist features. Lot 2 suits firms providing strategic advice, implementation support and expertise in areas such as risk stratification, social prescribing, medicines use and policy impacts including housing, transport, employment and the environment. Lot 3 suits providers able to deliver integrated end-to-end solutions. Relevant preparation includes interoperability, data integration, configuration, training, change management, service levels and performance reporting. NHS SBS also highlights sustainability and social value, and SAP Ariba registration requires a valid DUNS number.

## Notice

This procurement covers the Population Health Management framework. The framework will cover population health management software solutions that enable analysis and management of health data from a variety of sources to improve healthcare delivery and decision-making. This will empower Approved Organisations and stakeholders to deliver proactive, personalised care while improving efficiency and supporting preventative health interventions. Also can support addressing health inequalities and the wider social determinants and support the decision-making process across the health and social care sector. The framework is divided into three lots: Lot 1: Population Health Management Digital Solutions Delivery of population health management systems that will support the NHS's and other public sector organisations desire to deliver population level health and care across whole, or large-scale areas of the population - for example, an integrated care system, a city or town, a health community, a large acute Trust or a local authority - or multiples of these. Lot 1 is suitable for solutions with the below characteristics: * Solutions that can address large scale populations, however defined. * Multiple features and functionality, covering a wide range of Population Health Management. * Typically draws data from a range of sources of other organisations. * Horizontal integration to provide large populations to manage. * May or may not include the features and functionality of more specialist solutions. Lot 2: Consultancy &Support Services The nature of services will be to address a variety of needs through providing information, advice, direction and potential solutions to customers. Suppliers will be expected to be able to assist customers to make more informed decisions surrounding their population health programme. * Support of the delivery of population health management to the NHS and broader public sector. * Consultancy services are about helping the NHS or other public sector organisations that may use this framework to implement and deliver population support for some or all of the people who live in the areas they either commission or provide health and care services to. * Consultancy could extend across the NHS, wider health, public health, local government, broader policy areas impacting on health (such as transportation policy, housing policy, employment, environmental - other areas of policy relating to population health management and social determinants of health. * Consultancy might also be more targeted, dealing with a more limited area of healthcare, such as risk stratification, social prescribing, medicines usage, etc. Lot 3: Combined Solutions This Lot covers the provision of Population Health Management solutions that deliver bundle services or comprehensive workflows. Population Health Management provided under this Lot should ensure a seamless, integrated approach, delivering all the necessary components required for Approved Organisations. Across all lots, suppliers must provide: * Software licences or subscriptions * Implementation, configuration, and integration services * Training, onboarding, and change management support * Ongoing maintenance, updates, and technical support * Clear service level agreements and performance reporting

NHS Shared Business Services Limited (NHS SBS) act in an Agency capacity for and on behalf of its customers (Approved Organisations) - existing and new. These comprise of NHS and Social Care organisations (whether acting individually, or on behalf of, or together as members of any consortia) along with any other public or private sector bodies which NHS SBS authorises to use the resulting Framework.

NHS SBS intends to put in place a Framework under the Procurement Act 2023 for the provision of Population Health Management and related Goods and Services for Healthcare and wider public sector to be used by NHS SBS Approved Organisations.
NHS SBS is undertaking early market engagement to inform the design, scope, and commercial structure of the Population Health Management Framework. The intention is to understand current market capability, innovation, interoperability approaches, pricing models, and barriers to entry, as well as to test potential lotting structures and service specifications.
NHS SBS is committed to ensuring that the Population Health Management Framework supports Approved Organisations and stakeholders to deliver proactive, personalised care while improving efficiency and supporting preventative health interventions. Through analysis and management of health data from a variety of sources to improve healthcare delivery and decision-making.
As part of this engagement, NHS SBS invites suppliers to participate in a preliminary market engagement exercise, which may include a supplier questionnaire, virtual supplier briefing session, and optional one to one discussions. Feedback gathered will support the development of the final specification, procurement strategy, and route to market. Participation in this engagement is voluntary and does not form part of any future evaluation process.
