## Summary

Berkshire Healthcare NHS Foundation Trust intends to commission a frailty-focused proactive care service in West Berkshire. The service will support an agreed cohort of people, primarily from identified population-neighbourhood groups, with selected additional patients where clinically appropriate. Its purpose is to improve outcomes and help keep patients out of acute hospital settings. The provider will work within the Berkshire West neighbourhood and Integrated Neighbourhood Team model alongside primary care, community services, social care, mental health, therapy and specialist services. Activities will include population-health-based identification and prioritisation, holistic frailty assessments, care-plan development and review, referrals into appropriate pathways, multidisciplinary team participation, coordinated decision-making and continuous monitoring of high-risk cohorts. This is a health service procurement.

The procurement is at the planning stage: the Trust has stated its intention to award under the Provider Selection Regime’s Most Suitable Provider process, rather than invite tenders under the Public Contracts Regulations 2015. The buyer expects the next sequential publication, an award notice, on 21 October 2026. The planned service is expected to operate from November 2026 until 30 September 2027, with the wider requirement described as commencing in autumn 2026 and aiming to deliver improved outcomes from winter 2026. Decision-making will involve the Deputy Chief Executive, Chief Operating Officer, Chief Financial Officer and Director of Finance. The procurement is structured as one lot covering the Berkshire West frailty service. The published information sets out the intended approach and service scope for market awareness and preparation.

This will suit providers experienced in proactive frailty care, neighbourhood healthcare and preventing avoidable hospital admissions. Strong candidates are likely to be able to assess and manage people with varying levels of frailty, use population health data and local intelligence to identify high-risk cohorts, and deliver proportionate holistic assessments, including mini-CGA or an equivalent structured assessment where appropriate. The service calls for clinical judgement, care planning and crisis management, including advance care planning, end-of-life planning and ReSPECT documentation where relevant. Providers should be prepared to coordinate referrals across community, specialist and multidisciplinary pathways, participate in regular MDT meetings and shared decision-making, and work effectively with NHS, primary care, social care, mental health and therapy partners. Organisations should use this planning stage to assess their capability and readiness before the intended award process.
