## Summary

WM5G Ltd is buying a configurable digital discharge workflow platform for NHS partner organisations. It will coordinate the full hospital discharge-to-home pathway, from patient identification and referral through triage, enrolment, discharge preparation, monitoring and completion. The scope includes the software, implementation, integration, UK cloud hosting, training, support and maintenance. The platform is intended to replace fragmented supplier systems and manual administration, giving staff a shared operational view of patient journeys and enabling management of referrals, tasks, consent, communications and service performance. It must integrate with existing Home Readiness, Remote Monitoring and Asset Management services and support onboarding across multiple providers and care settings. WM5G invited offers through an open competitive flexible procedure. The process has reached the tender stage. The single-lot contract has an estimated value of £450,000 excluding VAT (£540,000 including VAT). Enquiries were due by 2 October 2026 at 14:00 and tender submissions by 21 October 2026 at 12:00. The stated contract term runs from 7 December 2026 to 6 December 2029, with an option to extend by up to two years, to 6 December 2031. Evaluation allocates 80% to quality and 20% to commercial price, using a staged assessment that includes pass/fail requirements, full tender evaluation, system demonstrations and dialogue, and final clarification. Up to three suppliers proceed to demonstrations and dialogue. The first-year budget ceiling is £250,000; a tender priced above £300,000 for year one may be considered unaffordable and excluded.

Experience delivering implementation, data integration, training, ongoing support and maintenance will be relevant. Suppliers must be able to demonstrate compliance with DCB0129, DCB0160, DTAC, DSPT, Cyber Essentials Plus and UK GDPR. Small and medium-sized enterprises are considered suitable to compete. The buyer also expects suppliers to demonstrate system capability, functionality, usability, integrations and self-configuration processes.

WM5G Ltd is seeking to procure a configurable Digital Discharge Service (DDS) Workflow solution to support the coordination of hospital discharge-to-home pathways across NHS partner organisations. The solution will provide a single operational platform through which WM5G Ltd staff can manage referrals, patient workflows, consent, tasks, communications and service performance, replacing fragmented processes that currently rely on multiple supplier systems and manual administration. The procurement covers the software platform, implementation, integration, hosting, training, support and maintenance required throughout the contract term. The solution will act as the operational orchestration layer for the wider DDS service, providing a unified view of patient journeys while integrating with existing Home Readiness, Remote Monitoring and Asset Management services. It is intended to improve the efficiency, scalability and governance of discharge services while maintaining clear accountability and visibility across multiple organisations and care settings. The platform must support the complete discharge workflow, from patient identification and referral through triage, enrolment, discharge preparation, discharge, monitoring and completion. Core capability areas include referral and onward-care management, workflow and task management, consent management, reporting and analytics, administration and role-based access control, standards-based integration, and data quality management. WM5G Ltd has defined a mandatory high-level architecture that suppliers must conform to. The solution must support API-first integration, HL7 FHIR interoperability standards, UK cloud hosting, strong identity management and secure integration with external systems. The architecture is designed to support rapid onboarding of new customers and providers while minimising operational complexity and vendor lock-in. The authority's desired outcomes are improved patient flow, reduced discharge delays, better patient and carer experience, reduced administrative burden, improved operational oversight, higher data quality and a platform capable of scaling across multiple regions and thousands of patients. Compliance with NHS and regulatory standards, including DCB0129, DCB0160, DTAC, DSPT, Cyber Essentials Plus and UK GDPR, is mandatory. Contract award will be based on the Most Advantageous Tender (MAT), assessing suppliers' ability to meet both the mandatory architectural requirements and the operational outcomes defined within the Requirements Compliance Matrix.

Lot 1

Options: The Authority may, at its sole discretion, procure additional services, deliverables or functionality related to the Contract during its term. Such options may include, but are not limited to, additional system integrations, configuration and workflow development, training, data migration, reporting enhancements, consultancy, technical support, or other professional services not included within the initial scope of requirements. Where applicable, these services may be commissioned on a day-rate, fixed-price, or other agreed commercial basis using the rates set out in the Contract. The inclusion of these options does not guarantee any minimum volume or value of additional work.

Renewal: The Contract will be awarded for an initial term of three (3) years, with the option for the Authority to extend for a further up to two (2) years. Any extension will be at the Authority's sole discretion and subject to satisfactory supplier performance, continued operational requirements, and the achievement of value for money. The extension may be applied to ensure service continuity and provide sufficient time for future service planning and procurement activities.
