## Summary

Bradford District Care NHS Foundation Trust commissioned a community key worker service for children and young people aged 0–25 with learning disabilities and/or autism in West Yorkshire. The service provides dedicated key workers to assess needs, engage with children and families, coordinate care, advocate and manage cases over time. It brings together NHS services, local authorities, education providers and voluntary, community and social enterprise partners. The initial contract term is three years, with options for two further 12-month extensions. The evaluation weighted quality and innovation at 30%, value at 25%, integration, collaboration and sustainability at 20%, inequalities and choice at 15%, and social value at 10%.
