## Summary

NHS Norfolk and Suffolk Integrated Care Board is developing a system-wide cardiovascular disease prevention and optimisation service across Norfolk and Suffolk. The proposed service will provide a digitally enabled pathway, initially focused on high blood pressure and cholesterol management. It is intended to connect identification and assessment with clinical review, starting and optimising treatment, behaviour-change support, monitoring and follow-up. The approach aims to replace fragmented, reactive activity with coordinated care, support national priorities for hypertension and cholesterol, and promote proactive population health, neighbourhood delivery and care closer to home. The service is expected to use existing local health system resources alongside additional capacity, applying consistent standards across the area while adapting delivery to local population needs, including underserved and deprived communities.

The process is at planning and the buyer has not started a competition. The ICB is using a second market-engagement event to update prospective providers and test its emerging service model and requirements before preparing procurement documents. It expects to commission one lead provider, accountable for coordinating and delivering the full pathway, with a consortium or subcontractor network where specialist expertise is needed. The stated estimated value is £12.4 million. The buyer expects to publish a further notice in June 2027. A face-to-face engagement event is planned for 27 November 2026 at 10 am in the Rose Mead Room, Endeavour House, Ipswich; organisations are invited to request a place by email. Attending will not confer an advantage in any later competition, and non-attendance will not prevent participation.

The planned model is likely to suit a provider able to lead a complex, integrated health service across Norfolk and Suffolk, bringing together clinical care, digital support, behaviour-change interventions, monitoring and follow-up. The lead organisation will need the capacity and governance to coordinate an end-to-end pathway and work with specialist partners through a consortium or subcontracted network. Relevant experience in cardiovascular prevention, hypertension and lipid management, population health and neighbourhood-based care would align with the stated aims. Providers should be prepared to deliver consistently across a broad area while adapting services to local needs, including those of rural, coastal, deprived and other underserved communities. The buyer has developed a proposed model and activity assumptions and is seeking market feedback before finalising its requirements, giving potential partners a chance to consider how they could contribute.

## Notice

The ICB is developing a single, integrated and digitally enabled CVD prevention and optimisation pathway across Norfolk and Suffolk, with an initial focus on hypertension and lipid management. The programme is intended to replace fragmented, reactive activity with a coordinated pathway that connects identification, assessment, clinical review, treatment initiation and optimisation, behaviour-change support, monitoring and follow-up. The pathway will support delivery of the Cardiovascular Disease Modern Service Framework priorities for hypertension and cholesterol, while also supporting a broader shift towards proactive population health management, neighbourhood delivery, care closer to home and reduction of health inequalities. The proposed service will bring together existing system assets and additional capacity. It is expected to support consistent standards across the ICB footprint while allowing delivery to be tailored to local population need, including Core20PLUS5 populations and deprived, rural, coastal and other underserved communities.

### Lot Information

Lot 1

The ICB intends to commission the service through a single Lead Provider arrangement. The Lead Provider will hold overall accountability for coordinating and delivering the end-to-end pathway and will be expected to work through an appropriately governed consortium and/or subcontracted network where specialist capabilities are required. Additional information: The initial PIN 2026/S 000-164159 was published in July 2026
