It developed into a specific Team within the Leisure Department before transferring to the Adults, Health and Well-being Department during June 2018 and became a part of the Well-being Team. The Team was originally funded under the ICF, but has now been funded by the Council for two years. Gwynedd Council's Cabinet asked for a formal evaluation of the Team in order to consider long-term funding.The CommissionThe successful applicant will be required to use the headings below to create an evaluation report for Gwynedd Council's Adults, Health and Well-being Management Team.Sum of money available to create the evaluation: PS3,5001.
