The Provider will be required to work in partnership with the individual, the City Centre Social Work Team to identify and refer on to appropriate lower support accommodation including the general Housing waiting list, extra care provision or private rented accommodation. The care and support practices involve caring about and for an Individual's physical, intellectual, emotional, psychological and social requirements, and should do so in ways that maintain dignity, are as unobtrusive as possible, and seek to ensure freedom from discomfort, boredom, insecurity, anxiety and isolation.
