Aneurin Bevan University Health Board (ABUHB) requires a contract for the provision of supported Living Service for Ford Road. Ford Road Bungalow in Fleur de Lys, Blackwood was purchased by First Choice Housing Association, working in partnership with ABUHB to develop bespoke accommodation for individuals with a Learning Disability diagnosis. The property is planned to provide a locally based home for two individuals who require 24hr support. The property is scheduled to be completed July/ August 2024. The service will support core services in offering specialist support for people who require intensive case management to function as independently as possible within their own accommodation. Typically, this patient group will have complex needs which traditionally would have been delivered in more secure settings (Residential settings, Locked Rehabilitation Wards, Low Secure Units). There are 2 individuals already identified for the scheme, person A is currently a delayed discharge on an acute ward, with length of stay of 850 days, whilst person B is currently in a private secure hospital having resided in a hospital setting for 30 years. Both individuals are placed outside their home Local Authority of Caerphilly.

Renewal: 3 years (26th November 2024- 26th November 2027) Option to extend for further 2 years (1+1) in two 12 months individual increments. 26th November 2027- 26th November 2028 26th November 2028- 26th November 2029
