The core purpose of care at home is to support people, and where appropriate their carers, to remain in their own homes, living as independently as possible with a good quality of life. We aim to ensure that care at home: - supports people to live well in the community - prevents people with significant health or care needs from having to use emergency services or be admitted to hospital inappropriately - helps people with care needs to look after themselves in the community The range of services to be provided includes personal care and support designed to: - help individuals sustain, recover or manage their health and abilities; - lead independent and fulfilling lives; or approach the end of life in their own homes where this is their choice..
