Targeted outcomes: 1. To prevent people becoming homeless wherever possible; 2. To prevent people being homeless any longer than necessary; 3. People show improvement in their independent living skills. Crisis Outreach Service and Support Hub Procurement of a service to provide crisis outreach and a support hub for people who are rough sleeping / who are homeless / at risk of homelessness who may have multiple and complex needs. Targeted outcomes: 1. To prevent people being homeless any longer than necessary; 2. People can access appropriate and suitable accommodation that will provide for their immediate accommodation needs, but also help them with transition to settled accommodation; 3.
