Project particular responses are required to the following questions in the ESPD 1. Question 4A.1 - if required by the member state, bidders are required to be enrolled in the relevant professional or trade registers within the country in which they are established. 2. Question 4B.1 - Bidders will be required to have an average yearly turnover of 200,000.00 GBP for the last three years. 3. Question 4B.5 - It is a requirement of this contract that bidders hold, or can commit to obtain prior to the commencement of any subsequently awarded contract, the types and levels of insurance as indicated below; Employer's Liability: 5,000,000.00 GBP Public Liability: 5,000,000.00 GBP Contractor's All Risk: Min 5,000,000.00 GBP 4. Question 4C.1 - Bidders will be required to provide two examples of works carried out in the past five years that demonstrate they have the relevant experience to deliver the work as described. 5. Question 4C.8a - Bidders will be required to confirm their average annual manpower for the last three years. 6. Question 4C.8b - Bidders will be required to confirm the number of managerial staff for the last three years. 7. Question 4C.9 - Bidders will be required to demonstrate that they have (or have access to) the relevant tools, plant or technical equipment to deliver the requirements of the project. 8. Question 4C.10 - Bidders will be required to confirm whether they intend to subcontract and, if so, for what proportion of the contract. 9. Question 4D.1 - Health and Safety Procedures: The bidder must hold a UKAS (or equivalent) accredited independent third party certificate of compliance in accordance with BS OHSAS 18001 (or equivalent) or have, within the last 12 months, successfully met the assessment requirements of a construction-related scheme in registered membership of the Safety Schemes in Procurement (SSIP) forum OR, comply with the requirements detailed in Clause 4D.1.4 of the Related Statements to the ESPD (Scotland) Questions document attached to this notice. 10. Community Benefits: Under the terms of this contract the successful supplier(s) will be required to deliver Community Benefits in support of the authority's economic and social objectives. Accordingly, contract performance conditions may relate in particular to social and environmental considerations. The Council seeks Community Benefits proposals from suppliers. The Community Benefits proposals provided by the supplier will form part of the contract award criteria and will be evaluated when determining the winning tender. See attachment in Additional Documentation 11. Non-Involvement Declaration: Falkirk Council will liaise with The Police Service of Scotland in order to assist one another in the prevention of organised crime through the procurement process. The Police Service of Scotland has identified that this is a business area which may be attractive or vulnerable to exploitation by serious organised crime groups. Therefore, as part of your tender exercise obligations, you are required to complete a declaration of Non-Involvement in serious organised crime. See attachment in Additional Documentation. 12. 24 Hour Call-out Service: The Bidder will be required to provide details of a 24 hour emergency call-out service to be provided for the duration of the contract.
