## Summary

Falkirk Council is seeking bids for the "Provision of a Short Term Supported Discharge Service", a project aimed at delivering rehabilitation and support services within the community. This procurement falls under the services category, specifically within the healthcare industry, and is currently in the Tender stage. The submission deadline is set for 25th October 2017 at 14:00, with an overall estimated contract value of £600,000. The service will cater to residents in the Falkirk area, focusing primarily on assisting service users at risk of long-term care or requiring discharge from hospital.

This tender provides a viable opportunity for SMEs and businesses specialising in care services, rehabilitation, and community support to expand their footprint in the public sector. Companies with experience in collaborating with healthcare professionals and delivering home-based assessments would be particularly suited for this contract. The ability to demonstrate suitable qualifications and meet rigorous care standards, including Care Inspectorate registration, will be crucial for potential bidders looking to secure this contract.

## Notice

The first strand Service is a short term rehabilitation, reablement and assessment service for Service Users at risk of requiring long term residential/nursing care or to prevent hospitalisation. The second strand is to promptly facilitate the discharge from hospital of in-patients or those presenting for emergency care who have been assessed as clinically fit for discharge but who may, following a further home based assessment, be found to have a need for reablement or rehabilitation and a requirement for short term or longer term support in order to remain at home. In both cases the Provider will work closely with the Service User, family carers, Occupational Therapists, Physiotherapists, other health professionals and known community networks to facilitate a support plan that allows Service Users to remain within their homes.
