To provide accessible accommodation based step down services in Orkney to enable earlier discharge from hospital or avoid hospital admission, with local co-ordination of flexible volunteer support in a homely setting. To provide barrier free accommodation for up to 6 weeks for discharged patients who live in rural and island locations, who live alone or are awaiting completion of adaptations to their home or re-housing and rehabilitation needs. To provide an opportunity for patients to access support services from the Intermediate Care Team, Occupational Therapy Team and other services and undergo rehabilitation / reablement programs in a homely environment. To provide a volunteer supported service with local co-ordination to enhance the opportunity for person centred, preventative care.

Renewal: The initial term of the contract will be 12 months, there is an option to allow an extension for a further 12 months (subject to the availability of funding).
