## Notice

The service to individual Service Users will be based on an individual care and Support Plan which has been prepared by the Care Manager following a Support Assessment.Individual Service Users needs will be assessed and specified by the Council. Any change to service provision must be approved by the relevant Council Leader. The service provided should promote independence and choice for the Service User while at the same time ensuring that the service meets all assessed needs and outcomes for the Service User.The service will accommodate varying levels of service provision within agreed parameters as outlined in care and Support Plans.The Service Provider will provide care and support in relation to health and social wellbeing. Appendix 1 provides an illustration of the tasks but this is not exhaustive.The Service Provider will ensure that this range of services is provided in a seamless manner in a way that meets the holistic needs of the Service User.The Service Provider will require to have the capacity within the service to be able to provide services at peak times during the day e.g. morning, mealtimes, bedtime, as well as providing specific and sometimes specialised services to individual Service Users, not only at agreed time, but also (within the overall parameters of the service) at unpredicted times, for example to facilitate discharge from, or prevent admission to, hospital.Care management will be the responsibility of the Care Manager, however, the Service Provider will co-ordinate the delivery of care and support. The actual service delivery will be subject to discussion and negotiation with the Service User and the Care Manager to ensure that, wherever possible, Service Users' preferences are accommodated.The Service Provider will ensure ongoing risk assessment is undertaken with respect to the service being delivered.
