Lot 1

The Wellbeing Service will be embedded in GP practices with the aim of supporting people with or at risk of long term conditions or common mental health problems, to live well. The service will provide 1:1 and/or group support to people who are facing challenging issues (including social issues) that affect their health and wellbeing. People will also be supported to access resources available in their communities. Practitioners will work as part of the primary care team and will work in close partnership with a wide range of community based services within Midlothian.
