Lot 1

Greater Glasgow and Clyde Health and Social Care Partnerships (HSCPs) wish to commission a specialist support service for people affected by Huntington's Disease (HD). The HD Specialist service is expected to provide a person-centred, flexible, family-orientated model of care which provides support throughout the many years which people may live with the condition. Key roles of the HD Specialists include holistic assessment, care planning and coordination, assisting individuals to navigate the health and social care system, emotional support and provision of information and advice about HD to patients and those involved in caring for them The tender is expected to deliver services across all six HSCPs. The changes to primary care service delivery, including the newly developed GP clusters, may provide opportunities for closer working with primary care practitioners as well as Mental Health service practitioners, and the successful bidder will be expected to work closely with HSCP managers in ensuring that the HD specialist service can flex appropriately to take advantage of opportunities to improve on service delivery. It will be the responsibility of the successful bidder to manage the HD specialist service flexibly across the city to ensure that there is consistency of support offered, including management of waiting times where this becomes necessary. It is expected, however, that waiting times will be kept within reasonable limits (up to 4 weeks maximum prior to first contact) and that robust management of the presenting caseload, will ensure that support is delivered in such a way that everyone eligible is able to benefit and receives support appropriate to their and their caregivers' needs. Referrals can come from a variety of sources including self-referrals, acute hospital staff, GPs and Mental Health Teams. The bid document should indicate the proposed referral pathway to the HD specialist service.
