Community Link Workers

The Glasgow City Health and Social Care Partnership is seeking dynamic organisations or partnerships of organisations, to deliver community led health and wellbeing programmes that will be key component of it's Primary Care Improvement Plan and complement its strategic direction for health improvement and the work of its own staff and many partners via: Individual and group support in GP practices/clusters/primary care setting through the Community Link Worker programme Community Link Workers will be located within primary care settings as part of a GP practice or cluster. Work will flow from the practice(s) but the CLW will also provide support to clusters they are attached to. (See Appendix 4 for Job Description.) The programmes will work mainly with adults aged 16 and over and their families in areas of deprivation, along with population groups who experience significant social and health inequalities. The programmes will have the fundamental aim of providing support and building capacity at both individual and community levels. Programmes may be focused at the levels of GP practices/clusters, geographic communities or targeted at specific population groups. Successful suppliers will be able to work collegiately, with existing partnerships and in forming new partnerships. It is expected that flexible, culturally appropriate approaches will be used and that the tenderers will demonstrate a sophisticated understanding of the pattern of inequalities in Glasgow and of the barriers in working in communities where people experience multiple vulnerabilities including poverty in many forms alongside loneliness and isolation. Successful suppliers will be responsible for areas including (but not exclusively): recruitment, line management of posts, and implementation of programme delivery. They will required to have relationships with the Commissioner (NHSGGC), GP practices (where staff will be located and work will flow from), GP Clusters and also a supplier delivering the Learning and Support package for Community Link Workers. This is a complex operating environment.

Children's Neighbourhoods Scotland Local Co-ordinators

Children's Neighbourhoods Scotland (CNS), a partnership between the Glasgow Centre for Population Health (GCPH), the University of Glasgow, Glasgow HSCP and Glasgow City Council, working with public, private and third sector partners, are seeking dynamic organisations or partnerships of organisations, to deliver the local implementation of CNS. Local Co-ordinators, as an integral part of the wider CNS team, will lead, deliver and support the community engagement and capacity-building aspects of CNS within the neighbourhood with children and young people, families and local stakeholders via: Neighbourhood approaches to improving outcomes for children and young people via the Children's Neighbourhoods programme. Children's neighbourhoods are focussed on improving outcomes for children and young people, through to school-leaving age. The approach is place-based and has the empowerment of children, young people and communities at its core. It seeks to join up efforts and services within a neighbourhood to prevent and reduce child and family poverty. Children's Neighbourhoods Scotland Local Co-ordinators will be based within a local anchor organisation, and will form a network led by, and accountable to, the CNS Programme Director based in the Glasgow Centre for Population Health (see Appendix 5 for Job Description). They will be required to develop strong relationships with a range of public, private and third sector organisations within the neighbourhood as well as with wider community planning partners and relevant national bodies. Children's Neighbourhoods Scotland will be evaluated by a team based at the University of Glasgow and receives core funding as part of the national Tackling Child Poverty delivery plan. Successful suppliers will be able to work collegiately, with existing partnerships and in forming new partnerships. It is expected that flexible, culturally appropriate approaches will be used and that the tenderers will demonstrate a sophisticated understanding of the pattern of inequalities in Glasgow and the West of Scotland and of the barriers in working in communities where people experience multiple vulnerabilities including poverty in many forms alongside loneliness and isolation. Successful suppliers will be responsible for areas including (but not exclusively): recruitment, line management of posts, and implementation of programme delivery. They will be required to have relationships with the Commissioner (NHS GGC, GCPH, and University of Glasgow). This will be a complex operating environment.
