### Lot Information

Care at Home

Providing a personalised care at home service to adults and older people that helps to meet the individual's agreed outcomes. The support may include personal care, social care/emotional support, aspects of health care and food and nutrition.

Housing Support

Providing a personalised housing support service to adults and older people that helps to meet the individual's agreed outcomes. The support may include assisting the individual to be safe at home and in the community, maximising their health and daily living skills so they can live as independently as possible, helping them to access and communicate with other relevant professionals/agencies to maintain their health and wellbeing. The service also includes encouraging and helping the individual to pursue social, leisure, learning and employment opportunities and providing assistance around personal budgeting/banking.

Respite/Short Breaks

Providing a personalised respite/short breaks service to adults and older people that helps to meet the individual's agreed outcomes. The service may include periodic respite, short-term respite and short breaks.
