## Notice

As this is a new Service, some flexibility of the Provider's part is expected.

Lot 1

The contract will provide early intervention person-centred support through a Community Link Worker and Wellbeing Practitioner Service to adults (aged 18 to 65 years) with moderate Mental Health problems to assist them in managing their symptoms and improving their wellbeing. Support will be provided via two community hubs (in Bathgate and Livingston), and in all West Lothian GP Practices. The Services will act as an alternative to the individual seeking support from their GP. Services will be delivered through partnership working between Primary and Secondary Care Practitioners, and the Third Sector.

Renewal: The Authority will be able to extend the contract for a period of two twelve month periods.
