Providing an innovative, flexible, person centred facilitation service, based on individual assessed needs that will include:- - Specialist advice, information, assessment and facilitation; - One to one group facilitation that will enable people with physical disabilities to identify and work towards outcomes that will enhance and improve social inclusion; - To identify existing local community opportunities such as employment, education, training, volunteering opportunities and leisure; - Maintain their database in place that signposts users onto other specialist resources, such as courses at local colleges; - To work with other agencies and individuals to develop activities in relation to aspirations, for example finding and or changing employment; - To work with others in a multi-agency approach to develop appropriate therapies or treatments, e.g. D&G Council and NHS D&G; - The service will not include providing any personal care on a regular or planned basis. However, the provider will ensure staff are able to risk assess, and when appropriate assist with personal care in emergencies; The cost of accessing activities and events will be met by the service user. The provider will meet any such costs incurred by their own staff when service users need to access activities. Indicative budget 144k GBP per annum. The contract duration would be for a maximum of four years. Currently, there are around 120 service users.
