### Lot Information

Lot 1

Provision of post-diagnostic support (PDS) following a dementia diagnosis is essential to inform and enable people with a diagnosis, and those who support them, to live as well as possible with dementia and prepare for the future. It is important the right support is offered at the right time following diagnosis in a person centred and flexible way. Delivery of this Contract is intended to assist the Orkney Integration Joint Board to meet the current Scottish Government LDP target. Post-diagnostic support within the scope of this Contract is for people newly diagnosed at an early stage of dementia where there is potential ability to develop some self-management skills to continue to live well with dementia.

Renewal: At the sole discretion of the Authority there will be the option to extend the contract by 2 12-month periods.
