Our priorities include helping with transitions for young people who have complex needs such as learning disabilities, severely challenging behaviour and other issues which lead them to being vulnerable. We are aiming to ensure that they live in a safe and supportive environment, build their independence skills and help them to become effective contributors to society.

### Lot Information

Lot 1

Over the course of the young person education, there will be child's planning meetings held which will address the young person's future needs and outcomes that are to be met. These meetings will include the young person, the teacher, families and relevant professionals. In 2017, The Scottish Transitions Forum produced a document called 'Good transitions 3'. The Principles of Good Transition are: - Planning and decision making should be carried out in a person-centred way - Support should be co-ordinated across all services - Planning should start early and continue up to age 25 - All young people should get the support they need - Young people, parents and carers must have access to the information they need - Families and carers need support - A continued focus on transitions across Scotland To build on this strategy, the City of Edinburgh Council proposes to adopt the following five principles as an improvement plan: - A single point of contact - Starting Transitions work earlier - Provide information to young people and families - Provide improved accommodation options - Ensure improved communication approaches and is seeking to create an open framework agreement to enable this to be taken forward. Therefore the City of Edinburgh is looking to commission services which support: - Young people to remain at home by offering day support or day and night time support - Young people to live independently, either with or without accommodation. Note: by submitting your interest you are agreeing to further engagement with the Council. The Council may decide to competitively tender this contract in the future. If authorised, this service may be awarded as a 4-year framework commencing 1st September 2020 to 30th August 2024.
