The aim of the Service shall be to provide a range of community based support focused on individual services to people from West Dunbartonshire over the age of 18 who have a varying complexity of support needs, which may include health needs. The Service Specification for this Service sets out the requirements relating to adults in the client care groups aged 18 years and over with physical or neurological conditions; acquired brain injury; enduring mental ill health; learning disability/ difficulty or life limiting conditions. It will support individuals to participate and live within the community.
