## Notice

Outcome - Participants will have enhanced evidence-based knowledge and skills to support the assessment and management of patients and their families living with Diabetes

Lot 2 Minor illness

Outcome - Practitioners will have enhanced evidence-based knowledge and skills to triage, manage and refer patients presenting with minor illnesses

Lot 3 Minor injuries

Outcome - Practitioners will have enhanced evidence-based knowledge and skills to triage, manage and refer patients presenting with minor injuries

Lot 4 Sexual Health

Outcome- Practitioners will have the evidence-based knowledge and skills required to deliver safe and effective sexual and reproductive health care in community and primary care settings.

Lot 5 Long Term Conditions Generic

Outcome - Practitioners will have the enhanced evidence-based knowledge and skill to confidently lead a long-term conditions service within a primary care setting
