The service to be provided will include (but not limited to): - Information and Advice; - Stress management therapies; - Promotion of wellbeing; - Providing access to social opportunities; - Facilitating Supportive peer Groups; - Counselling; - Facilitating access to Health and Social care support; - Supporting Carers through hospital discharge; - Facilitating and Accessing short breaks; - Financial Maximisation; - Training; - Completion of Adult Carer Support Plans; - Completion of Young carers Statements.
