The Service will be flexible and designed to meet the needs of carers in Aberdeenshire. ### Lot Information Lot 1 Aberdeenshire Council, on behalf of the Health and Social Care Partnership and Education and Children's Services, are seeking to commission a range of supports and resources for both adult and young carers. The Service will aim to improve the quality of life for carers through an outcomes led approach, providing the right level of intervention, at the right time. The Service will be flexible and designed to meet the needs of carers in Aberdeenshire. Key Service Aims: - Identification of carers through contact with the Health and Social Care teams, Education and Children's Services, other third sector organisations and development of innovative ways to engage with hidden or marginalised carers; - Provide carers with information, advice and enable them to access resources within their communities; - Support carers to complete either an Adult Carer Support Plan or Young Carer Statement; and where this is declined, record and plan other support that is available through the service - Support carers to complete either an Adult Carer Support Plan or Young Carer Statement; and where eligible refer to the carers team for an SDS budget.
