The level, nature and content of the individual care and support plans will be based on the assessed needs of each individual and take full account of their wishes and aspirations and their Guardians, families / carers within a financially sustainable model. Both projects accommodate individuals with complex needs who rely on continuity of care and support, any change of provider or staffing would have a hugely detrimental impact on these individuals and could be very destabilising.
