Notice Description

The aim of the digital diabetes champions project is to ensure that nobody is left behind. Be that with their achievement of their three treatment targets (blood pressure, cholesterol and HbA1c) or to the growing options of online diabetes services like Know Diabetes (KDS), which offers online behaviour change support and access to a patients test results for the 300,000+ people living with diabetes in NWL, and REWIND a remission programme for type 2 diabetes and virtual group consultations. This project aims to use existing digital champions (based in NWL) to reach a total of 200 people with diabetes to access online diabetes services, as well as basic digital skills to help manage their condition. This is of particular relevance given the introduction of COVID-19, which has highlighted the issue of digital exclusion seen by an estimated 22% of the population in North West London.
