Notice Description

Developing a national tool that captures the top 'high impact changes' to support health and social care systems to improve discharge, assessment, and commissioning of care in the community for those with dementia and delirium. A high impact change model (HICM) is a flagship tool of the PCH programme, within which the BCF Support Programme sits, with each model focusing on a theme which is essential to the successful delivery of integrated care and health locally. While the content of each HICM naturally differs, the models are consistent in synthesising the evidence base into a clear framework that describes 'what good looks like', with practical 'top tips', I/We statements, supporting resources/publications case studies, self assessments, checklists, powerpoint presentations/discussion points. The buyer is looking to secure a supplier with relevant experience and expertise to produce a HICM for publication on improving the experiences of discharge, assessment and care within the community for those with dementia and delirium. The content of this HICM should be consistent with the other LGA models, and also include a detailed road map/enablers. Additional information: The contracting authority considers that this contract may be suitable for economic operators that are small or medium enterprises (SMEs). However, any selection of tenderers will be based solely on the criteria set out for the procurement.
