prevention of social isolation and improvement of quality of life for people living with dementia in Kensington Chelsea & Westminster through the provision of arts based volunteers, arts projects & partnership programmes Resonate Belong monthly newsletter with events listings & activities plus 6 monthly 'in touch calls' Resonate Connect a one to one volunteer service (Creative Befrienders) visiting people at home /taking them to theatre, galleries, museums etc Resonate Together group art events, projects & programmes with Resonate Associate Artists and/or partner arts organisations Contributing to: Dementia Pathway; Dementia Friendly Comunities; Social prescription options for plwd; joint working Additional information: A new contract is put in place for a period of 18 months to afford the newly forming NHS North West London CCG time to agree the approach to commission this service at NWL ICS level in line with national and ICS guidance to ensure smoother, less complicated, non-duplicated service delivery.
