Notice Description

The Supporting Wellbeing (SWIFt) projects have been running for 2 years as part of Time to Shine, managed by Leeds Older People's Forum and funded by Big Lottery to tackle loneliness and social isolation. The project was set up to test models which work in preventing loneliness in older people, with SWIFt particularly focussing on providing older people living with frailty additional support to improve their wellbeing and independence. The service has been comprehensively evaluated in terms the impact it had on individual's wellbeing, levels of social isolation and usage of health and care services. Learning from the model has been reviewed to develop, expand and continue to test a future model for the service. From April 2019 Leeds City Council assume responsibility for commissioning the service. From October 2019 the service (outlined in section 3) will be broken down into 3 elements as below; 1. Continuation by Existing Delivery Partners; 2. 6 Additional Local Care Partnership areas and 3. a City Wide . Element 1 of the service are the subject of this agreement.
