Community Dementia Support Services
Notice Description
Essex County Council is seeking to establish a framework with a single provider to develop and deliver a service for people living with dementia and their families and carers across Essex. The service will work in partnership with professionals across the health and social care system in Essex, whilst taking into account local differences, to provide seamless access to coordinated information and advice to enable people living with dementia, and those that care for them to livewell, and independent for as long as possible. The service will support people living with Dementia, their families and carers to access community based information and support. The service will support them to navigate the system and build community assets between the elements, the community, system and local implementation.
