"description": "Contract Award Justification: * Improving access to healthcare, * Working in partnership with both primary care and Hospitals to improve access to the range of pathways for ill health prevention, * Health promotion and community treatment options. * When a patient requires hospitalisation the projects support efficient and effective safe discharge protocols that ensure smooth transition back in to the community and reducing the likelihood of readmission. * Ensuring there is a strong VSC offer strengthens our local community It is anticipated that this not only improves health outcomes for this vulnerable group but may also reduce costs incurred by avoiding extended hospital stays and improving patient flows.",
