Notice Description

NHS Midlands and Lancashire Commissioning Support Unit, on behalf of South East Staffordshire & Seisdon Peninsular CCG & Cannock Chase CCG is looking to commission 24 hour Community care packages for four individuals who have a learning disability and complex health needs; for example, epilepsy and restricted mobility. Service Users may have behaviours which challenge and / or have a forensic history. Service Users may be subject to legal restrictions such as a Community Treatment Order. Services will provide long term support for these clients in either, supported living settings, or in their own homes. These patients are currently in- patients identified on the transforming care cohort, requiring repatriation back to Staffordshire, to this end housing solutions will be required. Care providers are encouraged to deliver care in the least restrictive environment with integration into the community; care providers are encouraged to work in partnership with local housing providers/ associations to provide bespoke packages of care and individual housing solutions, it is expected that the care provider will be the prime contractor.
