Notice Description

NHS Central London, NHS West London and Hammersmith and Fulham Clinical Commissioning Groups (the 'CCGs') are working together to develop a new local service to provide outreach support to young carers under 18 years of age, who provide care, assistance and support to another family member who may be disabled, physically or mentally ill, or has a substance misuse problem, to deliver the strategic aims of reducing health inequalities and improving specific health outcomes for affected families. The service is intended to provide support to over 300 families across the London boroughs of: Hammersmith and Fulham; Kensington and Chelsea; and Westminster. The scope of service will include: visiting families in their home where a young carer is involved in providing support; offering whole family interventions; improving outcomes through a whole family approach. Challenges for the young carers and families being supported include: housing; mental health; income; prevention of illness; self management of existing long term conditions. Whilst an initial service specification has been drafted, intending to support a procurement process to award a 3 year service contract worth cira PS120,000 per year, EXPRESSIONS OF INTEREST ARE INVITED from potential service providers. Along with your confirmed expression of interest, the CCGs welcomes your feedback and comments in regard to the draft service specification as well as any initial thoughts you may have in relation to how such a service could be delivered, approaches to co-producing the service, what assets might be harnessed and any other added value, including social value might potentially result.
