Public Health Locally Commissioned Public Health Services (LCPHS) NHS Health Checks
Notice Description
Nottingham City Council are seeking suitably qualified and experienced providers to deliver the Locally Commissioned Public Health Services (LCPHS) NHS Health Check programme to eligible citizens living within Nottingham City Boundaries. Please note that contracts will be awarded to all suitably qualified and experienced providers and the application process will remain open for the three year duration of the contract, 1/4/23 - 31/3/26. The National NHS Health Check programme aims to help prevent heart disease, stroke, diabetes, kidney disease and certain types of dementia. Everyone between the ages of 40 and 74, who has not already been diagnosed with one of these conditions, will be invited (once every five years) to have a check to assess their risk and receive support and advice to help them reduce or manage that risk. Providers will be expected to produce a list of patients who are eligible for the NHS Health Check programme, using the NHS Nottingham & Nottinghamshire Integrated Care Board's eHealthscope Health Check tool, or an alternative compatible IT solution. Financial payment for the programme will be activity based as follows. * Providers will be paid PS45.00 per NHS Health Check completed for an eligible priority group patient. * Providers will be paid PS10.00 per NHS Health Check completed for an eligible non-priority group patient. * A bonus payment for first invitations sent to at least 70% of the annual eligible population is also available - PS5.50 x 70. This contract will be subject to a Light Touch Monitoring Regime, in line with the Public Contract Regulations 2015. The contract will remain open to new applications for the duration of the lifetime of the contract. The contract will span a 3 year period from 1st April 2023 to 31st March 2026. To express an interest please provide evidence that, as a minimum, your organisation meets the competencies set out in the NHS Health Check Competency Framework. These competencies comprise of: a) Core competencies (Care Certificate or equivalent). b) Clinical skills competencies specific to the check. c) NHS Health Check programme competencies.
