Care Home Coordinators in Croydon
Notice Description
One Croydon Alliance Programme Management Office is inviting expressions of interest from suitably experienced and capable providers, to provide a coordinating and facilitating role to enable the recruitment, development and oversight of a brand new team of Care Home Coordinators who will work across the London borough of Croydon. Funding of up to PS285,000 has been secured to run this new service as a pilot for at least 12 months, with the aim for the new Coordinators to act as the link between "Integrated Community Networks Plus" (ICN+)* Locality Teams and local care homes, joining up care across the system in Croydon. This new team of Coordinators will also provide care plans for people in the community predominantly for people over the age of 65. The coordinating provider will be specifically invited to employ and manage 6 (six) ICN+ Care Coordinators during this pilot phase. *Note: the ICN+ is a major programme of transformation and integration that will improve outcomes for Croydon people through a proactive and preventative approach within each of the six Localities of the Borough. It focuses on all adults and is aligned with services for children and families. The rollout of the ICN+ integrated community teams has already started to see positive outcomes for Croydon residents. The new ICN+ Care Home Coordinator role is being introduced to further enable the model to be sustainable and to proactively support residents to stay well. Current commissioning intentions assume that a contract for the new pilot service can be awarded during May 2022 to enable recruitment and ramp-up of the service during the summer of 2022. The expected budgetary allocation for this pilot service will be up to PS285,000 for 12 months. Or, please explain what might be preventing your interest at this time (including what might have made this opportunity sufficiently more attractive to your organisation). 2. Please provide a succinct summary/introduction about your organisation, outlining what services are currently provided, including ideally those having particular regard to the borough of Croydon. 3. If your organisation was to become involved in delivering this service, would this be through a direct provision by your organisation, or would you intend to partner aspects of the service either as a lead provider or a sub-contractor? Please explain as appropriate, including details of any organisations (or simply type of organisation) you might work with. If this would be your intended approach and interest, how would you envisage the joint provision be delivered? 4. The One Croydon Alliance Project Management Office has a current working assumption about the timeline to mobilise this service (gradually ramping up between 1 to 3 months) but would invite you to set out what you would assess to be realistic and feasible, and why. 5. What outcomes would you expect to achieve through this service? ***PLEASE RESPOND ASAP by 12noon, Wednesday 19th January 2022. Provider organisations responding as invited above, to confirm their interest, are likely to be invited to engage further with the One Croydon Alliance PMO. Note, whilst your response is not a pre-requisite to any future procurement opportunity, responses, or lack of them will be used by the One Croydon Alliance PMO as evidence and inform selection of the most appropriate process to award a contract(s), including whether a competitive process should be used.
