Notice Description

This is a Request for Information (RFI) published on behalf of NHS North East Essex Clinical Commissioning Group (CCG). With this RFI North East Essex CCG is seeking to learn the level of possible interest from organisations in relation to the provision of an Autism Spectrum Disorder Service (ASD) for young adults aged between 16-19 years and up to 25 years for those with Special educational needs and disability (SEND). There has been a great deal of research that this group of individuals and between the age of 16 to 25 years old, have very little or no access to services that could provide a great deal of support, for them to progress into education or employment. The overall outcome of the service will be to reach a cohort of young people with diagnosed or non-diagnosed autism who have struggled to access a variety of existing services which are individually tailored to support the young person with a Person-Centred approach. Key areas of support will include: - Health and Wellbeing - Education - Employment - Family Support - Life Skills. The CCG is seeking to commission a high quality, timely and accessible service to registered young people with GP practices in North East Essex that will focus on outcomes in line with ASD NICE guidelines. The successful organisation will provide a digital solution in a range of formats (telephone, video etc) to receive real-time requests from health and care professionals/parents and young people for; advice, information and guidance and will provide a suitable pathway into appropriate clinical and non-clinical services to support young adults to achieve the outcomes set out above. The process will require there be an audit trail and that monthly reporting details a summary of month and YTD performance.
