Notice Description

The contract has been awarded to The Confederation, Hillingdon CIC following market engagement to invite capable providers. The Confederation, Hillingdon CIC has been established as the only capable provider. The proposal is to support and encourage patients on the GP SMI register who DNA or decline SMI health checks, vaccine and screening. This will be done buy a Social Prescriber Link Worker who would work across the 6 primary care networks supporting with engagement of SMI patients through outreach work. A Social Prescriber Link Worker Will enable people to have more control over their lives and signposts to further support and community groups. They also will help to reduce healthcare inequalities by supporting people to unpick complex issues affecting their wellbeing and help them to engage via a series of contacts with the individual. The Social Prescriber Link Worker, would work closely with GP practices and MIND to identify appropriate patients specifically those from black and minority ethnic groups who need more intensive support for them to engage. Ideally the individual will be able to engage the patients in their mother tongue and understand cultural barriers. From local data we can see that the highest level of prevalence are in the south of the borough where we know there's a high proportion of the population from black and minority ethnic groups. By working closely with GP practices, they will also support with educating practice staff and helping to promote local mental health training undertaken by MIND.
