The Self-Care programme provides support to patients over 65 accessing primary care, to give them the knowledge, skills and confidence to improve their own health and wellbeing and manage their long-term health conditions. The following services are delivered under this contract by 8 local Voluntary & Community Sector organisations (sub-contracts); Additional information: A new contract is put in place for a period of 18 months to afford the newly forming NHS North West London CCG time to agree the approach to commission this service at NWL ICS level in line with national and ICS guidance to ensure smoother, less complicated, non-duplicated service delivery.
