Notice Description

This contract Award Notice is for the purpose of transparency only. To deliver outcomes to service users and other stakeholders. This contract will deliver Specialist Support Work, under the Fluctuating Needs criteria. The service will: Plan and deliver outcomes focused, person centred support to service users. Provide a level of support which is appropriate to the needs of the service user, with a focus on independence and personalised outcomes. Reduce dependency on support provided by paid carers. Safeguard Vulnerable Adults including robust support planning, risk assessment and risk management. Work in partnership with service users, health and social care professionals and other stake holders to ensure that effective communication with service users is central to service delivery. (This might include the use of communication systems such as Makaton, PECS or British Sign Language and the use of assistive / augmentative technology). Monitor the delivery and quality of support and review support arrangements where applicable. Work alongside the service user, their family and circle of support, health and social care professionals and other stakeholders to develop co-produced services. Support service users to build social capital by leading active, healthy, socially responsible and socially inclusive lives as part of their local communities. Maintain a socially inclusive approach to support, which seeks to develop the use of local community networks and opportunities for service users. Use proactive and non aversive strategies to support service users to manage their emotional wellbeing and reduce behaviours which may limit life opportunities. Support service users to make informed choices about their life, promoting self advocacy. Support service users to take positive risks and manage identified risks in an enabling way. Support service users to look after their home and belongings and to manage home ownership, a tenancy or licence to avoid eviction and repeat homelessness. Act as an advocate, or enable those accessing the service to self-advocate, in the resolution of problems that have negative impacts on the stability of Service Users' housing or future housing options. Support services users to maximise their income. Support service users to manage personal money and a household budget. Support service users to be aware of their financial responsibilities and support access to services such as credit unions, rent deposit schemes and to establish debt and savings plans. Support service users to plan and prepare meals and drinks where appropriate. Support service users with personal care, including the provision of direct support if this is required. Support service users to keep healthy, manage medication and interact with Health professionals. Provide support for Palliative care interventions where required and within appropriate commissioning arrangements.
