Notice Description

Brighton & Hove City Council is seeking to commission a service provider to design and deliver an integrated Ageing Well Service (AWS) for older people in Brighton & Hove to start on 1 April 2019. The AWS will be open to anyone aged 50 years and over but will target those older people who are identified as being most at risk of a decline in their independence and wellbeing. The service will deliver innovative primary prevention and health promotion to support people to age well in Brighton & Hove. The service will: o Reduce social isolation o Reduce loneliness o Promote good health and wellbeing, o Prevent ill health o Enable older people to remain independent for as long as possible Potential providers should note that a contract is to be awarded for a period of 4 years with the option to extend the period of the contract for further periods up to a maximum of two years in total at the sole discretion of the council. The council has a maximum budget available for delivery of the contract requirements of ?2,392,000 over four years of the initial period.
