Notice Description

The aim of the Community Delivery of NHS Health Checks is to appoint a single provider to reduce the risk of Cardio-Vascular Disease (CVD) by individually assessing everyone in the target group, communication the risk and offering lifestyle advice and other interventions to reduce their risk. By addressing the main risk factors for CVD, this will contribute to the prevention of heart disease , stroke, diabetes, kidney disease and dementia. This new community contract is required to reach the eligible population who do not received health checks through their GP practice, reaching more people across County Durham and covering any gaps in service delivery.
