SPS 2597 - Discharge to Assess
Notice Description
The Royal Borough of Greenwich is seeking to commission a provider to deliver a Discharge to Assess (D2A) within the Borough. This service focuses on patient flow and the facilitation of early discharge by ensuring that assessing a patient's long-term care needs is not done in an acute setting. The service should consist of no more than ten beds that will primarily take referrals from QEH (not excluding other hospitals) with the gatekeeping function led by the Hospital Integrated Discharge Team in partnership with other relevant professionals. The ethos of the assessment unit will be to enable partnership working to support residents to achieve individual goals as set out in their discharge to assess care plans. Once patients are in the D2A unit, they will be holistically assessed to determine their long-term care and support needs by a multi-disciplinary team. For example, could they return home with a care package, be referred to Extra Care Sheltered Housing or require a long-term care home placement. Discharge to assess is not specific to any category or level of care; people accessing this service may require residential or nursing care, including dementia care. For example, the hospital might have indicated that someone requires double-handed assistance; however, during their time in the discharge to assess bed, they may improve and would require single-handed support. Providers will work with Council Officers to deliver care that is strength based and outcome focused, with a strong emphasis placed on changing the way that homecare is used and delivered; with providers progressively changing, in line with Local Authority changes, throughout the life of the contract. Providers will also have the opportunity to deliver care in to one of three Extra care schemes within the Borough as well as deliver homecare to the neighbourhoods in which these schemes are located. The Contract period is for 3 years. Additional information: Price is not the only award criterion and all criteria are stated in the procurement documents. This procurement will be managed electronically via the RBG Proactis Procontract etendering suite. To participate in the procurement, participants shall first be registered on the e-tendering suite.
