Notice Description

NHS North West London ICB requires the provision of HIV Specialist Community Service. This service is to be provided to the 8 North West London boroughs. The aim of the service is to provide specialised & individually tailored treatment for individuals with HIV who, have a cognitive impairment, HIV-related brain injuries or complex physical rehabilitation needs. The service is currently being delivered by Mildmay Hospital. The ICB's view is that Mildmay Hospital is the only capable provider to deliver a service covering all NWL residents & supporting Neuro-cognitive impairment (HNCI) as well as complex physical rehabilitation for patients with HIV in the appropriate facilities (20+ beds to facilitate stays of 30 days+). The ICB seeks to be transparent in its intention to award a further 12 month contract from 1st May 2023 to Mildmay Hospital up to a total estimated value of PS208,000 (with 50% funded on a block & the remainder cost & volume based). Recognising the historic trend with small patient volumes of activity fluctuating year by year we intend to ensure some tolerance for under/over performance is reflected in the contract. The form of contract used will be the NHS Standard Contract 2023/24 particulars (shorter-form). Requirements for participation: -Appropriate facilities with minimum requirement of 20 beds & ability to facilitate 30 days length of stay or over. -The inpatient unit will provide physiotherapy, occupational therapy, psychological therapies, speech & language therapies, dietician services, psychiatry, counselling, social care & chaplaincy. Following admission, all patients with cognitive impairment will have a 28-day assessment to affect a comprehensive review of rehabilitation needs. -The service is to offer integrated assessment & rehabilitation services which can be delivered on an inpatient & outpatient basis. -Cater for HIV patients requiring the service are those with complex physical rehabilitation needs & HIV Associated Neurocognitive Disorder (HAND). Capable providers may come forward by 17:00 on 25th April 2023 via: nhsnwl.contracting@nhs.net - Suppliers must evidence appropriate facilitates & staffing arrangements. FURTHER DETAILS PROVIDED BELOW. Additional information: CONTINUED: Other requirements: -Assessing immediate & primary symptom management needs & treat as appropriate with constant evaluation & review. -Developing an individual plan of care for the individual in consultation & agreement with the Clinical Nurse Specialist & the key stakeholders. -Customising a comprehensive & coordinated package of care for patients. -Ensuring the patient's care team, (medical, nursing, social worker & care-giver) are consulted on relevant decisions about the care of the individual & provided with an updated at discharge. -Offer dedicated services to patients who meet the acceptance criteria. Patients suitable for the service will have access to a merged service for Neuro-cognitive impairment (HNCI) & complex physical rehabilitation. The service provider should ensure: -Clinical staff join & maintain membership with their own specialist HIV related affiliated groups as well as more generic groups whose work could or may influence best practice. -The service should continue to provide an effective specialist service for people with HIV by ensuring that all clinical staff are up-to date within their own specialism & also with changes to care & legislation affecting people with HIV. The ICB is committed to assess the needs of the service users & consider how it needs to secure the needs of the people who use the services, & improve the quality & efficiency in the provision of the services including through the services being provided in an integrated way.
