Notice Description

In the first instance Provider will provide a minimum of 350 Registered Nurse hours per week for the first 6 months. We anticipate that the number of hours may vary by approximately 10-25% over the lifetime of the contract. This would be by mutual agreement through a quarterly review process. The Nurses will provide comprehensive care to named patients in their place of residence. In the first instance care will be provided between the hours of 8am to 6pm on a 7 day a week basis, exact distribution to be agreed in partnership. Over an agreed period of time the intention will be that the Nurses will work in line with the agreed shift patterns for Neighbourhood Teams. The Trust will align the Nurses to a Neighbourhood Team who will identify the work to be undertaken. In time the Nurses will be aligned to a caseload cluster within the Neighbourhood Team and will work in line with the Neighbourhood Team model of care. Additional information: Leeds and York Partnership NHS Foundation Trust (LYPFT), (the "Authority") is issuing this invitation to quote ("ITQ") on behalf of Leeds Community Healthcare NHS Trust (the Contracting Authority) whom is seeking a Provider with the appropriate product range, experience and competitive pricing to supply it with Medical Care at Home Patient Discharge Services. The Leeds Community Healthcare NHS Trust is looking to appoint a provider Patient Discharge Services Medical Care at Home until 31/03/23 with an additional optional 1 year extension period. SBS Patient Discharge Services Framework SBS/18/AP/RIG/9258 Lot 3 providers only. Prospective bidders in the further competition must meet the quality and price criteria of the ITQ document.
