THIS PROCUREMENT IS OFFERED IN 2 (TWO) LOTS: - LOT 1 EXPERT PATIENT PROGRAMME - LOT 2 DIABETES CHAMPIONS AND DIABETES USERS GROUP NHS Central London, NHS West London and NHS Hammersmith &Fulham Clinical Commissioning Groups (CCGs) intend to competitively tender the offer of a contract for the provision of an Expert Patient Programme Service, Diabetes Champions service and Diabetes Users Group service to be provided for its registered population across the CCGs' catchment areas (London boroughs of Hammersmith & Fulham, Kensington & Chelsea and Westminster). A formal procurement process is to commence on 10 June 2015 inviting tenders from interested potential providers to enable a contract to be awarded by August 2015 and for services to commence as soon as possible thereafter, expected to be by October 2015. The Contract is likely to be for three (3) years with an option to extend for a further two (2) years. The estimated contract value is PSXXm per year. ***EXPRESSIONS OF INTEREST are INVITED from potential providers. Note: the submission of an expression of interest is not a prerequisite to any subsequent procurement application. The Expert Patient Programme (EPP) is a structured 6-week course, delivered by Lay Tutors living with a Long Term Condition, in accordance with the Chronic Disease Self-Management Programme developed by Stanford University. The aim of the Expert Patient Programme (EPP) is to enable people with long-term conditions to: * Feel confident and in control of their lives * Aim to manage their condition and its treatment in partnership with health care professionals, e.g. GPs, Practice Nurses, Consultants * Communicate effectively with professionals and be willing to share responsibility of treatment * Be realistic about the impact of their condition on themselves and their family * Use their skills and knowledge to lead full lives The service is to be delivered within the Central London, Hammersmith & Fulham and West London CCG boundaries. The Diabetes Champions is a volunteer-led initiative to target local people with, or who are at risk of, developing Type 2 diabetes. The aim of the service is to help raise awareness of Type 2 diabetes and risk factors, and help people make better lifestyle choices. The Diabetes User Group provides a targeted forum to ensure that the voices of the local population contribute directly to the provision and development of local diabetes services.
