Notice Description

The service provides non-clinical Care Navigators to work in and alongside general practice to deliver a care coordination service, ensuring that patients at high risk of admission and / or with complex health and social care needs have a care plan in place (which is regularly reviewed) supported by proactive case management. Additional information: This interim contract award represents a fundamental redesign of the service. It is considered that the incumbent provider is best placed to deliver this new service in the short term while the CCG engages further with the wider market regarding a long term solution. Any longer term solution would also need to be incorporated into the CCG's commissioning plans for a Multi-Specialty Community. As such, a short term (9month) contract to test the new model has been awarded to an incumbent provider in order to enable the CCG to evaluate its outcomes and consider a long term solution.
