NHS West Cheshire CCG Wellbeing service - engagement event
Notice Description
NHS West Cheshire Clinical Commissioning Group (CCG) have developed a new service model to deliver a Wellbeing Service to patients with one or more long term condition, who are registered with practices in the West Cheshire CCG geographical area. The Wellbeing service is designed to support patients with one or more long term condition/s to resolve their non-medical needs and improve their effectiveness to self-manage their conditions. The service being tendered has two elements: 1 The provision of advice, signposting and support, and where necessary, low level non-statutory advocacy support, to assist them with a broader set of non-medical issues. 2 Deliver self-care management courses and one to one peer support. The aim of this service is to provide support, signposting and advice to improve the wellbeing of those patients with long term health conditions; to address social, and non-medical issues that are causing them to access health services inappropriately. As well as supporting those with long term conditions to improve the management of their condition. The Wellbeing Service will take direct referrals from GPs, other practice staff and community care teams and provide follow-up information and advice on a wide range of issues. Working with a 'Wellbeing Coordinator' the patient will build personal resilience, and improve their ability to self-manage their own health and wellbeing thus reducing demand on GP/hospital/Integrated Health and Care services.The key service outcomes we are looking to deliver are: * Improvements in patients' mental health and wellbeing. * An increase in the range of services offered by the Practice, allowing a more holistic care package * Releasing General Practice appointments * Interventions to increase social participation * Patients feeling supported to increasingly self-manage their long term conditions * Reduction in A&E attendances among patients referred to the Wellbeing service * Improvement in the link between the Practice and the local voluntary sector. Commissioners are therefore inviting interested service providers to an engagement event to be held on Thursday 19 January 2017 from 1400 to 1600 at Burley Hall, Waverton, Chester, Cheshire CH3 7QN. The purpose of this event is to share with providers the vision for this Wellbeing service and we have attached the draft service specification for your information and review. Your feedback on the specification and involvement would be welcomed as part of the event. The other key elements of the event will be Finance and how the Procurement process will be managed. Invitations are limited to a maximum of 2 attendees per organisation. If you wish to attend, please send confirmation of attendee name, email address, role and organisation by email to Tony Ryan, Procurement Manager (anthony.ryan2@nhs.net). Registration for this event will close at 1600 on Tuesday 17 January 2017 Additional information: Please see attached draft service specification for your information and review as your feedback would be appreciated during the engagement event.
