Notice Description

Expressions of interest required:- Kent Community Health NHS Foundation Trust require a safe and efficient electronic system which fully enables parents, children and young people to complete a comprehensive, confidential health needs assessment questionnaire online. The system will need to provide a structured framework using evidence based practice. The health needs assessment tool/software needs to be effective to deliver a structured holistic health questionnaire throughout key life stages to ensure that the service can deliver health and wellbeing reviews as part of a comprehensive range of universal and targeted support to safely deliver the healthy child programme for school age children 5-19 years. The questionnaire will need to enable parents, children and young people to complete their questionnaires online via a desktop, tablet or phone. The questionnaires need to be framed to provide early intervention notifications that can be used to support individual needs and also provide data at a whole school approach level to enable the service to support the early identification of need to support wider public health agendas. There is a requirement that the system has the ability to highlight health issues and potential risk factors and direct the service users to steadfast resources to support and enhance their health and wellbeing. Expressions of interest (EOI) must be submitted via the Procontract procurement portal (Proactis) under Opportunity number DN548078. Any correspondence to the Authority's staff email address will be disregarded. Please note that no further documentation will be available until the ITT is published following closure of the EOI.
